ESR Blood Test Explained: What a High Sed Rate Means
The erythrocyte sedimentation rate (ESR, or sed rate) is one of the oldest blood tests for inflammation. It is simple and inexpensive but not specific: a high ESR says inflammation may be present, not where or why. This guide explains how to read it and when it matters.

Typical ranges at a glance
- Men under 50
- Up to about 15 mm/hr
- Men 50 and over
- Up to about 20 mm/hr
- Women under 50
- Up to about 20 mm/hr
- Women 50 and over
- Up to about 30 mm/hr
- Rule of thumb for the upper limit
- Men: age ÷ 2. Women: (age + 10) ÷ 2
ESR is reported in millimetres per hour (mm/hr). Ranges vary between labs; use the one printed on your report.
What does ESR measure?
ESR measures how far red blood cells settle to the bottom of a tall, thin tube in one hour. During inflammation, proteins such as fibrinogen make red cells clump together, so they fall faster and the ESR is higher.
Why normal ESR rises with age
ESR naturally rises with age and is usually higher in women. A result that would be high for a 25-year-old can be normal at 75. That is why labs and clinicians use age- and sex-adjusted limits.
Common reasons for a high ESR
Infections, autoimmune and inflammatory conditions such as rheumatoid arthritis and lupus, anemia, kidney disease, pregnancy and older age. A very high ESR, often above 100 mm/hr, can come with serious infection, inflammation of blood vessels, or blood cancers such as multiple myeloma, and always needs follow-up.
When a high ESR is urgent
In people over 50, a new headache, tenderness of the scalp or temples, jaw pain when chewing, or any change in vision together with a high ESR can be a sign of giant cell arteritis. This needs same-day medical care because it can threaten sight.
ESR vs CRP
Both measure inflammation. CRP rises and falls within days, so it tracks what is happening now. ESR changes over weeks and is also affected by anemia and blood proteins. ESR is still useful for following conditions such as polymyalgia rheumatica and giant cell arteritis.
What about a low ESR?
A low ESR is rarely a concern. It can be seen with a high red cell count (polycythemia), sickle cell disease or a very high white cell count, and is usually not followed up on its own.
Frequently asked questions
What ESR level is concerning?
It depends on age and sex, but results well above the age-adjusted limit, and especially above 100 mm/hr, need a search for the cause. A mildly high ESR without symptoms is common and is often simply rechecked.
Can a high ESR be normal?
Yes. ESR is higher with age, in women, during pregnancy and with anemia. A mild rise may have no significant cause, which is why it is read together with your symptoms and other tests.
Is ESR or CRP better?
Neither is better overall. CRP responds faster and is less affected by other factors, so it is often preferred for infection. ESR remains useful for some inflammatory conditions. Many clinicians order both.
Can ESR detect cancer?
ESR is not a cancer test. Some cancers raise it, but most high results have other causes, and a normal ESR does not rule cancer out.
