A symptom tracker that keeps the story for your next appointment
“When did this start, and how often does it happen?” is hard to answer from memory. VHT keeps a dated record of your symptoms next to your conditions, medications, sleep and activity, so you and your clinician can see the pattern.
- 33 common symptom types, the same list Apple Health uses
- Severity, body location and notes on every entry
- Link a symptom to a condition you already track, with its triggers

Log a symptom in seconds
Tap Symptom in the Quick Log bar, pick what you feel, from headache, fatigue and dizziness to heartburn, coughing or a racing heartbeat, and rate it as none, mild, moderate or severe. Add where it hurts and a short note if you like. You can also log it for an earlier day.
Connect it to a condition
If the symptom belongs to a condition you already have, such as migraine or asthma, link it and note what triggered it. If it is new and you do not know the cause, you can start tracking it as a new concern to raise with your clinician. VHT does not diagnose it.
See the pattern on a calendar
Every symptom appears on your calendar with a monthly chart, so you can see how often it happens and whether it lines up with poor sleep, a new medication or a stressful week.
Symptoms from Apple Health come in too
If you already log symptoms in the Apple Health app, VHT syncs them into the same record, so nothing is entered twice.
ECHNA helps you prepare
ECHNA can see your last 90 days of symptoms. Ask “What should I tell my doctor about my headaches?” and she can summarize when they happened, how severe they were and what else was going on, so you walk into the appointment with facts instead of guesses.
How to start a symptom diary
- Open the ECHNA screen in VHT and tap Symptom in the Quick Log bar.
- Choose the symptom and how severe it is.
- Add the body location, a note, and a linked condition if it fits.
- Check your calendar over the next weeks, and ask ECHNA for a summary before your appointment.
Good to know
- A symptom tracker records what you feel. It does not tell you what is causing it.
- Chest pain, trouble breathing, fainting, or sudden weakness or confusion can be an emergency. Call your local emergency number instead of logging it.
- If a symptom is new, getting worse or worrying you, contact a clinician. Do not wait for a pattern.
Symptom tracking: common questions
What is the best way to track symptoms for my doctor?
Log each symptom when it happens, with the date, how severe it was and anything that might have triggered it. A dated record is far more useful to a clinician than a description from memory. VHT keeps that record and ECHNA can summarize it before your visit.
Can VHT tell me what is causing my symptoms?
No. VHT and ECHNA help you record and understand your symptoms, and ECHNA can explain possible general reasons and when to seek care. Only a clinician can diagnose the cause.
Can I track symptoms for a chronic condition?
Yes. Link symptoms to a condition in your record, such as migraine, asthma or irritable bowel syndrome, and note the triggers. Over time you can see how often flares happen and what tends to come before them.
Does it sync symptoms from Apple Health?
Yes. Symptoms you log in the Apple Health app sync into VHT on iPhone, alongside the ones you log in VHT.
Is the symptom tracker free?
Symptom logging is part of Quick Log, which is included in every VHT plan, including the free Basic plan.
